Dentist Near Logan Square

Convenient Access from Logan Square

From the eagle-topped column at the center of Logan Square, Forward Dental Studio is about 5 minutes away by car. The practice occupies a storefront on Milwaukee Avenue in Chicago, near the southern edge of the same community area, so a dentist near Logan Square is within reach whether you drive, take the Blue Line or ride the bus. Dr. Luis Peralta, a prosthodontist, is the one dentist you will see here.

Leave the Illinois Centennial Monument heading south on Kedzie Boulevard, which carries on as Kedzie Avenue, and turn east onto Armitage Avenue. Follow Armitage to Oakley Avenue, swing south on Oakley, and you will meet Milwaukee Avenue right at the office. It is about 2.1 miles, with no tolls along the way.

By train, board at the Logan Square Blue Line station, 2620 N Milwaukee Ave, and ride two stops through California to Western. A walk of about 4 minutes finishes the trip. The #56 Milwaukee bus stops at the Logan Square station too and runs down Milwaukee past our door.

Rideshare drivers can use 1879 N Milwaukee Ave STE 1, Chicago, IL 60647. Google Maps will route you there from any corner of the neighborhood. If you live farther northwest, Avondale and Hermosa have their own pages.

Boulevards, Greystones and Your Logan Square Smile

The square itself is hard to miss. The Illinois Centennial Monument, a 70-foot marble Doric column crowned by an eagle, went up in 1918 for the state's 100th birthday, and its architect, Henry Bacon, also designed the Lincoln Memorial. It marks the meeting of Logan Boulevard, Kedzie Boulevard and Milwaukee Avenue.

From there the Logan Square Boulevards District spreads out along Logan, Kedzie, Palmer and Humboldt Boulevards, listed on the National Register in 1985 and named a Chicago Landmark in 2005. The community area runs roughly from the Metra Milwaukee District North Line on the west to the North Branch of the Chicago River on the east, and from Diversey Parkway down to the 606. It lies in Cook County, with Avondale and Hermosa among its neighbors.

Stately greystones and roomy bungalow-style homes line those boulevards. The community includes an established Latino population, Eastern European families and a growing number of young professionals, and the Logan Square Branch Library on Fullerton Avenue serves all of them. Milwaukee Avenue remains the main commercial street, and it is the same street our office faces.

Comfort First for Logan Square Patients Who Have Put Off Care

Some people delay the dentist for years because of nerves, a bad past experience or simple embarrassment. Our studio was set up with that in mind. Weighted blankets, noise-canceling headphones and personal TVs are available at every visit, and you can agree on a signal to pause whenever you need a break.

For appropriate patients, oral conscious sedation adds another layer of calm during longer treatment. A consultation decides whether it suits you, and you receive clear instructions for before and after the appointment. The Dental Anxiety & Patient Comfort page describes each option in more detail.

Owners of older Logan Square homes often arrive with aging crowns, worn teeth or a missing molar. Young professionals who ride the Blue Line downtown are more likely to ask about straightening or brightening their smile. Both groups tend to favor Saturday mornings, which start at 9:00 AM and wrap up by 2:00 PM; the Monday through Friday schedule runs 9:00 AM to 5:00 PM.

For certain cosmetic cases, the Smile Trial pairs digital planning with a 3D-printed preview worn over your own teeth. It lets you judge a proposed change in the mirror before any permanent step.

Smile Upgrades and Repairs from a Dentist Near Logan Square

  • Cosmetic Dentistry

Whitening, bonding, veneers and smile makeovers can be combined into one plan or done one step at a time. Both new and returning patients qualify for a complimentary cosmetic consultation to explore what is possible.

  • Porcelain Veneers

Custom porcelain can reshape chipped, gapped or uneven front teeth. As a prosthodontist, Dr. Peralta designs them around your bite as carefully as your shade.

  • ClearCorrect®

Removable clear aligners straighten mild to moderate crowding or spacing for teens and adults. Planning starts from a 3D intraoral scan of your teeth.

  • Dental Implants

For a lost tooth, an implant restores chewing strength while leaving the teeth on either side untouched. CBCT 3D imaging helps plan placement when the extra detail is useful.

  • General Dentistry

Checkups, cleanings and fillings are where every new patient begins, nervous or not. Digital x-rays come up on a screen so you can review them together.

The neighborhood's mix of longtime homeowners and newer arrivals means these services rarely stand alone. One patient might begin with a nervous first cleaning, plan an implant for a missing molar a few months later and finish with whitening so everything matches. With one prosthodontist guiding each step, the cosmetic and restorative pieces are designed to fit together from the start.

Meet Your Chicago Dental Team

Dr. Luis Peralta leads each new patient's first appointment himself and often performs the cleaning too. Two familiar faces complete a deliberately small team: Jesenia, who handles scheduling up front, and Tatiana, who assists chairside as a Registered Dental Assistant. Logan Square patients see the same three people every time they visit.

Insurance & Financing

Aetna is in-network, and Guardian is among the plans we accept via the Careington Network. For patients managing gum disease without insurance, our In-House Discount Plan offers a Periodontal Wellness Package at $55 monthly or $660 a year, with four periodontal maintenance cleanings and 15% off other services.

Frequently Asked Questions

How far is Forward Dental Studio from Logan Square, and how do I get there?

About 2.1 miles separate the Illinois Centennial Monument and Forward Dental Studio, roughly 5 minutes by car. The route follows Kedzie Boulevard south, then Armitage Avenue east and Oakley Avenue south to Milwaukee Avenue. Anyone looking at a dentist near Logan Square can also take the Blue Line two stops to Western.

How do I get from the Logan Square Blue Line station to your office?

Ride a downtown-bound train two stops, through California, and get off at Western. From there, walk southeast along Milwaukee Avenue for about 4 minutes to reach the office. If you prefer the bus, the #56 Milwaukee leaves from the Logan Square station and runs straight down Milwaukee past our door.

Can I have oral conscious sedation for a long appointment?

Possibly. For appropriate patients who need extra help with dental anxiety, oral conscious sedation is an option, and a consultation with Dr. Peralta decides whether it suits you. If it does, you receive instructions to follow before and after the visit.

What is the Periodontal Wellness Package?

It is the in-house plan tier built for uninsured patients who need regular gum maintenance. For $660 a year or $55 a month, it includes four periodontal maintenance cleanings, two oral exams, two fluoride treatments, all necessary x-rays and 15% off other dental services, and none of it comes with waiting periods or annual maximums.

My old silver-colored filling cracked. Can it just be refilled?

Sometimes, but not always. If enough healthy tooth surrounds the crack, a new filling may be enough; if much of the tooth has broken away or a cusp has snapped off, a crown usually protects it better. Dr. Peralta reviews the x-rays and a 3D scan with you on screen before recommending either repair.
